Evolve Practice Ltd Privacy Policy  

Evolve Practice Ltd (‘we’ or ‘us’ or ‘our’) gathers and processes your personal information. This Privacy Policy sets out to describe why we collect personal information, what we collect, how it will be used and who it will be shared with. Evolve Practice takes care to protect the privacy of its clients, gathering and processing your personal information in accordance with this policy, complying with all relevant data protection regulations and laws.  

Why we collect personal information.

We only collect and process personal information for the lawful purpose of business administration, including health administration, research, staff administration and accounting. Evolve Practice Ltd is registered with The Information Commissioners Office, (ICO), the number is  ZB622584.

We process information to support the provision of a healthcare service. This will also include the processing of associate therapists’ information. We may also use information to monitor and evaluate the service we provide; however, this type of information will usually be anonymised.  

Evolve Practice will never collect any unnecessary personal information from you and will only ever process your information in the way outlined in this policy. 

What personal information do we collect: – 

  • Identity data – Name, date of birth, title and gender 

  • Contact data – billing address, email address, telephone number, 

  • Insurance data – insurance number, claim number, insurer details 

  • Location data – your IP address and telephone codes 

  • Transaction data – card and bank transfer payments 

  • Technical / analytical data – IP address, web pages visited prior to coming to our website, information about your browser network and device, may include details about how you use our website e.g which pages you clicked into. 

  • Recruitment data – for our associates, including CV, identity and contact data, accreditation, DBS and insurance details.  

  • Therapy notes – your therapist records notes about your sessions, and stores these on our Cliniko system. These notes can be viewed by your therapist, and other practitioners who may be involved in your treatment at Evolve Practice.  

  • Group sessions – sessions are not recorded. The only information collected is the length of sessions time and attendance. Please note that group participants will be able to see you on screen and to see your first name only.  

  • Sensitive Data – We will collect information on health/medical, details about religion, racial and ethnic origin, sexual orientation.  

  • Service Evaluation data – we may process information on our outcome measures we use to assess progress in therapy.  

 

How do we collect your personal information 

We collect information provided by yourself via Zoom, email, contact form, cliniko and telephone consultation. This includes the completion of forms, questionnaires / measures, assessments, client contract and client feedback forms. 

 

We may also collect data automatically through our website by using cookies or other similar technologies. Please refer to cookies policy for more information.   

 

Third party information may be collected in the form of health data from GP or Psychiatrists if consent has been given. Third party information may also be received from insurance companies.  

 

How we use your personal information 

Evolve Practice takes your privacy seriously and we will never disclose your personal data without your consent, unless required to do so by law.  

Evolve Practice will only hold personal information for as long as is determined by our regulating bodies, this may vary depending on the individual clinician (therapist/ coach) 

.  

We process your personal data for the purposes of business related activities and to provide you with our services. Business related activities include website monitoring, anonymous therapy service evaluation. Service provision activities include registering you as a client of Evolve Practice, booking appointments, delivering treatment / coaching, processing self-funded payments and insurance payments, completion of forms/ surveys/questionnaires, and other communications. 

We may also process personal data to comply with our legal obligations, in accordance with law and regulations. Including our legal obligation to keep you safe, should a life threatening situation arise. 

If we receive a request from you to exercise any of the above rights, we may ask you to verify your identity before acting on the request; this is to ensure that your data is protected and kept secure. 

 

Who we will share your personal information with 

Evolve Practice may share information when there is an obligation to comply with current legislation or if you consent to sharing of information. 

We will never share your personal data with any third party for their marketing purposes. Our website may contain links to third party websites, we are not responsible for any third-party websites or data collection and always advice you read their privacy policy. 

Evolve Practice will only hold personal information for as long as is determined by your clinician / therapists regulating body.
